Preoperative Instructions

Please bring the following to your appointment:

• Referral
• Medical and dental insurance cards
• List of current medications
• Photo ID

If you are going to have a procedure, please fast 6 hours prior.

If you are going to be sedated, you are required to have your driver with you and you may not drive a car or operate machinery for 24 hours.

All patients under the age of 18 must be accompanied by a parent or legal guardian.

If you are taking a blood thinner (anticoagulant) other than Aspirin, have a heart valve or joint replacement (knee, hip, etc...), are taking medicine for Osteoporosis, or have had Radiation therapy to your Head and Neck please notify our office when scheduling your appointment.
